How many Americans have diabetes right now

The Centers for Disease Control and Prevention puts diagnosed diabetes at roughly 29 million US adults, or about 11 percent of the adult population. Add an estimated 8 million people who have the disease but have never been told so, and the true share climbs closer to 38 million. That is more than one in ten adults walking around with a condition that quietly damages kidneys, retinas and nerves for years before symptoms show up.

Those national figures hide enormous local variation. Colorado and Utah sit near 7 percent of adults; Mississippi, West Virginia and parts of the Deep South run above 13 percent. If you want a number that matters for your own community, the CDC’s PLACES dataset publishes diagnosed prevalence down to the census tract, and county health departments often break it out by ZIP code in their community health assessments.

The age curve is steep. Fewer than 3 percent of adults under 35 have diagnosed diabetes, while roughly 29 percent of those 65 and older do. That means the caseload will keep climbing even if incidence flattens, simply because the population is aging.

What a decade of data shows about the increase

Between 2014 and 2023, the number of diagnosed adult cases rose from about 22 million to roughly 29 million, a jump of nearly a third. Most of that growth came from three sources: an aging population, better screening that catches cases earlier, and rising obesity rates. Obesity prevalence went from about 35 percent to 42 percent of adults in the same window, and excess weight is the single strongest driver of type 2 disease.

The rate of new diagnoses, though, has actually leveled off. New cases among adults fell from 1.4 million a year in the mid-2010s to around 1.2 million recently, according to CDC surveillance. The honest reading is that we are diagnosing a growing pool of existing disease rather than creating a much faster wave of new patients. Youth diagnosis is the exception — type 2 among people under 20 has been climbing faster than in any other age group.

Where the numbers get misread

Headlines that claim diabetes has nearly doubled in ten years usually mix diagnosed and undiagnosed cases or compare raw counts without adjusting for population growth. A more careful comparison: age-adjusted prevalence went from about 10.2 percent to 11.6 percent over the decade. That is real and troubling, but it is a 1.4-percentage-point change, not a doubling.

Another common confusion involves the waistline-driven rise in prediabetes. About 98 million adults have prediabetes, and the vast majority have no idea. Only a fraction progress to full diabetes each year — roughly 5 to 10 percent — but the pool is so large that it drives most future growth.

Using the data in a real patient encounter

When obtaining a sample history from a patient with diabetes, the numbers are a useful frame, not a script. A 52-year-old in a county with 14 percent prevalence who mentions fatigue and frequent urination is a very different risk case than a 30-year-old marathoner with the same complaint. Knowing local prevalence changes what you ask about first — family history, gestational diabetes, weight trajectory, and prediabetes labs from the last three years.

The American Diabetes Association’s screening guidance has shifted toward earlier testing: all adults at any BMI with one risk factor, and everyone at 35 regardless. A hemoglobin A1c of 6.5 percent or higher on two occasions confirms the diagnosis; 5.7 to 6.4 lands in prediabetes territory. A single fasting glucose of 126 mg/dL is also diagnostic if repeated.

Cost is a practical barrier. Metformin runs about $4 to $10 a month on most pharmacy discount programs. Continuous glucose monitors, increasingly prescribed for type 2 patients, cost $40 to $75 a month with insurance but can approach $300 without. Insulin prices have fallen sharply since the 2023 cap for Medicare beneficiaries at $35 a month, but uninsured patients still face sticker shock.

Practical takeaways for households and clinicians

Ask your county health department for its diabetes prevalence figure. If it is above 12 percent, that usually means screening programs exist and are underused. The CDC’s National Diabetes Prevention Program runs in-person and online cohorts across most states; the year-long program typically costs under $500 and is covered by many employer plans and Medicare.

For clinicians, the most useful data point is often the trend line in your own panel. Comparing the share of patients with an A1c under 8 percent against the national benchmark — about 60 percent of diagnosed adults are at goal — tells you more than any state map. The rest is ordinary primary care: annual foot exams, retinal screening every year or two, urine albumin checks, and a statin for most patients over 40.

The diabetes increase is real, and it is concentrated in specific zip codes and age brackets. Knowing which one you are standing in makes the numbers actionable.
